Our Studio Policy and Standard Form of Agreement is a contract created by Musicians Union and The Incorporated Society of Musicians to provide clarity and structure for your lessons.

We ask for your cooperation in adhering to the guidelines outlined in this agreement, as they are in place to ensure a positive experience for both students and teachers.

Following your introductory lesson/period, we request that you review and complete this form, along with submitting fees for the upcoming series of lessons.
